Certified Home Care Services

Acclaimed Visiting Nurses is certified to provide home care services through Medicare. Most people over the age of 65 are eligible for Medicare. If an individual is under a physicians care, is home bound, and requires skilled services, he or she may be eligible for services provided by a Medicare-certified home care agency, such as Acclaimed Visiting Nurses.

Medicare will cover 100% of home care services if they meet the meet Medicare’s criteria. Many home care agencies will refer to their Medicare division as their “Certified” side. For more information on Medicare services and eligibility requirements, visit www.medicare.gov.

Medicare defines very specific roles for your home care agency and the care they provide. These roles are defined as follows:
  • Assessment: The Acclaimed nurse completes a comprehensive assessment of every patient they see. This assessment will involve areas of body systems, including vital signs; psycho-social assessment in areas of “coping” and “Adjusting”, outside support etc; environmental safety and nutrition.
  • Teaching: The home care nurse may teach in areas of self-care. It is the nurse’s task to help assess what areas a patient can provide their own self-care in, and what is the best way for them to learn self care tasks. Many times the nurse will be reinforcing teaching that already has occurred in the hospital or doctor’s office.
  • Specific Interventions: These are “interventions” that are driven by a patient’s diagnosis. These can be things like lab work or blood work, or wound care as a few examples.
  • Coordinate Care of other healthcare provider: The home care nurse will coordinate care with other providers of health care services. These services can include coordinating personal care assistance with a home health aide, physical therapy, occupational therapy, speech therapy, social work and ordering medical equipment. The nurse also communicates on a regular basis with the primary care physician and/or the physician who ordered the home care.
  • Ancillary/Other Services: Acclaimed nurses are a wealth of information on community resources. The nurse can often be the first step in setting up such services meals on wheels, transportation and private duty services.
Here are some examples of areas in which Acclaimed Visiting Nurses can provide certified home care services.
  • New diagnosis of Diabetes, Congestive Heart Failure (CHF) or multiple co-morbidities
  • High probability that medications will have to be frequently adjusted
  • Complex or multiple new medication schedule
  • Pain-severe or intractable
  • Patient has quickly de-conditioned and needs Physical Therapy and/or Occupational Therapy ( i.e., weakened and unsteady gait)
  • Unhealed wounds, cellulitis
  • Breathlessness when walking around
  • Post surgery- spinal fusion, joint replacement
  • Other common home care diagnosis
    • Osteoarthritis
    • Cancer
    • CVA
  • Patient would have difficulty leaving home unassisted.
  • Minimal family support or lives alone
  • Potential for non-compliance for treatment, especially when patient has difficulty understanding
