Making a Referral Acclaimed offers two easy ways to make a referral: You can call our office at 248-352-0100 Or you can fax a referral to 245-223-8494 An easy to use referral form that has been formatted with many check-off options is provided for your convenience. Just download the form, print, fill in information and fax. After a referral has been received: Acclaimed intake person disseminates information and appropriate nurse or therapist is scheduled to manage and provide the home care First visit or opening assessment is usually scheduled the day of, or the day after, referral is received. Time of discharge can have an effect on when patient receives first visit Home Care Nurse or Therapist will first call patient to introduce themselves let them know when they will be coming for the opening visit.
