Your Home Care Team
Home Care utilizes many different health care disciplines to deliver care and form the “home care team." Although each team member has a different role, they all work toward the same goal: helping patients recover to their optimal level. Your primary team members include:
Your Attending Physician, who has the critical role in determining whether home care is an appropriate care setting for
their patient. The attending physician writes orders for the service and approves the home care Plan of Care.Your Acclaimed Visiting Nurse, whose home visits provide a nursing assessment of patients in their home environment. She also can teach the patient and family members in self-care techniques, instruct on new or complex medication routines, and provide wound care, diabetic care, and other skilled services. She also “manages” the care and interaction of other members of the home care team. Based on need, other team members can include:
Physical Therapists, who help patients regain strength and mobility. They provide gait training, exercises, and
rehabilitation techniques in the comfort of home.Home Health Aides, who assist the patient with personal care, primarily bathing and dressing routines. Occupational Therapists, whose training and technique enable patients to regain independence. They train patients in the use of assistive devices and provide work simplification for routine household chores. They also utilize teaching and exercise to help patients regain fine motor skills. Speech Therapists, who provide voice and language disorder treatments, speech articulation and swallowing treatments. Medical Social Workers, who assist with identification of community resource and long-term planning, and counseling to maintain maximum independence. |
